TPA Software: Check the Claim Pack Before Adjudication

TPA software runs a health claim from pre-authorisation to settlement, but most of it records the decision instead of checking the papers behind it. That gap sits in the claim pack: the discharge summary, final bill, investigation reports, pre-authorisation approval and KYC that arrive from the hospital.
A Head of Claims at an Indian TPA now works against an IRDAI clock: one hour for a cashless request, three hours for discharge. On that clock, a missing page or a mismatched date becomes a query, a delayed discharge and sometimes a complaint. The fraud discussion gets the attention, yet most packs fail for plainer reasons.
KlearStack sits beside your claims system and checks each pack against the rules you set. The Claims Triage Agent screens the file and sends high-risk claims to a person with reasons. Your medical officer and your insurer keep every decision.
| TPA software (definition) TPA software is the claims administration system a third party administrator uses to service health insurance claims for insurers. It holds member and policy records, handles cashless pre-authorisation, processes hospital bills and tracks settlement. In India, TPAs work under the IRDAI (Third Party Administrators – Health Services) Regulations, 2016, and the insurer alone communicates a repudiation. |
TL;DR
- TPA software manages the claim workflow; it rarely checks whether the documents inside the claim agree with each other.
- IRDAI’s Master Circular of 29 May 2024 sets one hour for cashless decisions and three hours for discharge authorisation.
- The same circular moves document collection to insurers and TPAs, so a missing page is now your problem, not the patient’s.
- IRDAI reported 1,37,361 health insurance complaints in FY25, up 41 per cent, with claims and document disputes among the biggest issues.
- Faster adjudication is the wrong goal: a TPA that closes a claim on its own can be warned, as Heritage Health Insurance TPA was in April 2025.
- The claim pack pre-check confirms seven agreements before a medical officer opens the file.
- The Day-One Query Test shows how many of your queries were avoidable, using 20 closed claims.
- Start with last month’s closed packs and count what the checks would have caught.

What Is TPA Software, and Where Does It Stop?
TPA software is the system of record for claims administration: members, policies, hospital networks, pre-authorisation, bills and settlement status. It stops at the workflow. It tells a Head of Claims where a claim sits, but not whether the bill, summary and approval describe the same admission.
TPA claims management software was built to move a case, and buyers compare TPA claims processing systems on workflow, payer reports and hospital portals. The check on the papers stays with a processor reading PDFs under time pressure. Earlier work on claims process automation covers the workflow side; this page covers the document check that sits in front of it.
| Job | Claims system | Claim pack check layer |
| Member and policy records | Holds them | Uses them as the reference |
| Pre-authorisation | Routes and records it | Reads the approval as one document in the pack |
| Bill processing | Captures amounts | Recalculates line items, taxes and totals |
| Names, numbers, dates | Stores what was typed | Matches them across every document |
| Claim decision | Records the outcome | Never decides; routes with reasons |
| Evidence | Stores the files | Logs each check against each document |

The One-Hour Clock Turned Every Query Into a Compliance Risk
The IRDAI Master Circular on Health Insurance Business of 29 May 2024 sets the clock. A cashless decision is due within one hour, and final authorisation within three hours of the hospital’s request. The policyholder also need not submit documents: insurers and TPAs collect them from the hospital.
That second line changes who owns a missing page. A claims processor can no longer treat a thin pack as the patient’s delay, because document collection now sits on your side of the table. Insurers had until 31 July 2024 to put the systems in place, the Finance Minister told Parliament.
The pressure shows in the complaint data. That same reply counted 2,57,790 complaints on the Bima Bharosa portal in 2024-25, with 4,811 not resolved in time, or 1.87 per cent. The share was 0.39 per cent in the first half of 2025-26, so the clock is being measured and reported.
| 📊 1,37,361 health insurance complaints in FY25, up 41 per cent from 97,503 Claims refusal, settlement delays, partial payments and document disputes sat among the biggest issues. For a Head of Claims, each one starts as a page someone read too late. Source: IRDAI annual report data via CAalley |
The National Health Claims Exchange will not close this gap alone. An IRDAI health sub-committee was still discussing NHCX adoption in August 2026, and a fast wrong document is still a wrong document. Teams weighing document automation in insurance should separate reading the pack from checking it.
Faster Adjudication Is the Wrong Thing to Buy
The assumption is that TPA software should adjudicate faster, ideally with AI making the call. The reality is that adjudication is the one step you cannot hand to a tool. In India, it is not fully yours to begin with.
No claim can be repudiated without the insurer’s Claims Review Committee, under the same Master Circular. A 2025 IRDAI order adds the second half: repudiation is communicated only by the insurer, never the TPA. The Head of Compliance at any TPA should read that order in full.
| ⚠️ Warning IRDAI warned Heritage Health Insurance TPA on 9 April 2025 after a remote inspection. It had closed claims for lack of documents without adequate evidence of queries or reminders, sent repudiations directly and lacked proper record-keeping. The failure was as much about proof as about the decision. |
The pattern across audit cycles is consistent. Teams rarely lose an argument because a reviewer read a document badly. They lose because nobody can show what was checked, when, and who was told.
So a reviewed pack is not a checked pack, and a checked pack proves nothing unless the check was logged. That ladder of reviewed, checked and proven is what an audit trail adds to a claim file. KlearStack takes the drudgery; your team keeps the judgment and the final decision, and spends its time on the work that needs it.
The Claim Pack Pre-Check: Seven Agreements Before Adjudication
A claim pack pre-check is a logged, rule-based review of every document in a claim before a medical officer opens the file. It confirms the pack is complete and its documents agree, then sends only the real exceptions to a person, with the reason attached.
| Agreement | Papers compared | What a failure looks like |
| Completeness | Pack against the insurer’s document list | Investigation report named in the summary is absent |
| Identity | Claim form, KYC, summary, bill | Patient name or policy number differs across pages |
| Dates | Admission, discharge, bills, policy schedule | Bill dated after discharge, or a date outside the policy period |
| Arithmetic | Line items, taxes, totals | Claimed amount sits above the itemised sum |
| Authorisation | Pre-authorisation approval, final bill | Billed procedure or amount differs from the approval |
| First use | This pack against earlier claims you hold | The same bill or certificate backed another claim |
| Integrity | Each page on its own | Edited amount, cloned region, copied stamp or seal |
KlearStack runs the identity, date, arithmetic, first-use and integrity checks today, and enforces sign-off order on claims that match your rules. Completeness and authorisation rules differ by insurer, so confirm them on your own packs in the pilot. Whether the diagnosis justifies the treatment stays with your medical officer, and whether a rate is payable stays with your tariff tables.
Integrity is one check among seven, not the whole job. For the fraud detail, see how insurance fraud detection for claim documents flags edits and repeat bills. Document tampering detection explains the page-level view.

The Day-One Query Test: How Many of Your Queries Were Avoidable?
A query on a fact that was already in the pack on day one is a check you ran late. This test measures that share, and a Claims Operations Manager can run it in under an hour without any software.
- Pull 20 closed claims from last month that drew at least one query.
- Open the first query on each claim and note the fact it asked about.
- Check whether that fact was visible in the pack as first received.
- Mark it avoidable if the mismatch or gap was already on the page.
- Divide avoidable queries by 20, and note which of the seven agreements failed most.
What we see in document-heavy claims teams is that the first query usually names something the pack already showed. The processor found it, but found it at hour two instead of minute two. That timing difference is what the clock punishes, and what automated document verification changes is mostly when the check happens.
Here is an illustrative calculation, so replace the inputs with your own. A TPA handling 20,000 claims a month, with one in four drawing a query and 40 per cent of those avoidable, carries 2,000 avoidable queries. At 25 minutes across both desks, that is about 830 hours a month spent on checks that could have run at intake.
The Same Pack From the Hospital TPA Desk: File It Once
A hospital TPA desk is the team that files pre-authorisations, assembles the claim pack and answers queries from the TPA or insurer. A Revenue Cycle Head carries the same risk from the other side: every query delays discharge, and every deduction becomes a receivable. The pre-check works before filing as well as before adjudication.
- Confirm the summary, bill and reports show one patient, one policy number and one set of dates.
- Recalculate the bill before it leaves the desk, including package and non-package items.
- Check that every investigation named in the summary has its report in the pack.
- Match the billed procedure to the pre-authorisation approval, and flag any change for an enhancement request.
- When a query arrives, reply with the page reference and the logged check, not a fresh bundle.
That last step is where query replies are won. A desk that can point to the exact page and the check behind it closes the loop in one exchange. KlearStack sits beside your HIS, and on-premise deployment keeps patient documents inside your network, which matters under the DPDP Act.
What to Look For in TPA Software, and When a Check Layer Is the Wrong Buy
Choose the claims system on workflow, payer coverage and reporting. Then ask a separate question about the documents, because most evaluation scorecards never do. A COO comparing vendors can test both with the questions below on real closed packs, not a scripted demo.
| Question to ask | Weak answer | Strong answer |
| What happens to a mismatch? | A processor spots it | It is flagged with both values shown |
| Who makes the claim decision? | The AI recommends closure | A person decides, with reasons logged |
| Can you replay a closed claim? | Files are stored | Each check, result and sign-off is logged |
| Can it run on your own rules? | Standard rules only | Your insurer lists and rules are applied |
| Where does health data sit? | Vendor cloud only | On-premise or private cloud is available |
Before a pre-check, processors find gaps at hour two, queries go out in batches, and the reason for a decision sits across emails. After it, gaps surface at intake and a person handles exceptions with reasons attached. The record is ready when an insurer audit or an IRDAI inspection asks for it.
A check layer is the wrong buy in three cases. You need a claims system or member portal, you need NHCX connectivity, or your packs are few and simple. KlearStack does not do the first two, and for simple packs a disciplined checklist will do once you have sized your document non-compliance risks.
KlearStack has processed 150M+ documents in total across banking, trade and supply chain work. Claims pilots run with design partners on one closed claims month. Treat that pilot on your own packs as the evidence, not this page.
Conclusion
Most TPA software tells you where a claim is. Few tools tell you whether the pack inside it is complete, consistent and logged, and that is where queries, deductions and regulator findings begin.
The cost is no longer abstract. BCG and Medi Assist estimate Rs 8,000-10,000 crore of annual claim payouts leak through fraud, waste and abuse. For the person who signed the file, the Heritage order shows the other cost: a warning, a board discussion and a 90-day action report.
Check the pack before adjudication and keep the record of that check.
FAQ
What is TPA full form?
TPA stands for Third Party Administrator. In Indian health insurance, a TPA is an IRDAI-licensed company that services claims for insurers, including cashless pre-authorisation and claim processing. The insurer still carries the risk and communicates any repudiation.
Who is the largest TPA in India?
Medi Assist Healthcare is described in press coverage as India’s largest TPA. Rankings change with the year and the measure used, such as premium serviced or lives covered. Check the IRDAI handbook for current numbers before relying on a ranking.
What is the difference between a TPA and a claims adjuster?
A TPA is a company that administers claims for an insurer under an agreement. A claims adjuster is a person or role that assesses a specific claim, more common in general insurance and in US usage. In Indian health claims, a TPA’s processors and medical officers do the assessment work, while the insurer keeps repudiation.
What is the time limit to submit a claim bill to TPA insurance?
The limit depends on the policy, the insurer and the TPA agreement, so check the policy wording and the hospital’s empanelment terms. Many schemes ask for documents within days or weeks of discharge. For cashless claims, the insurer or TPA collects documents from the hospital, not from the patient.
